Solution
If you have a content management system (CMS) such as Wordpress or Joomla, you should use a widely used, well-praised plugin to add a contact form. For Wordpress, e.g. a popular, solid plugin is "Contact Form 7" by Takayuki Miyoshi. For Joomla, the extension "Contacts" is included in the basic installation. Typical alternatives are "Flexi Contact" by Les Arbres Design and "Breezing Forms" by Crosstec.
If you have programmed your website yourself, you must also develop the form functions yourself. However, there are many ready-made code snippets available for this purpose. You can find them using a Google search (for example "PHP contact form").
In any case, you should protect each form with a captcha from abuse. Bots will otherwise send spam through it.
